Multiple Disciplinary Team (MDT) Treatment Could Help Convert Unresectable HCC of Late Stage to Resectable

Introduction: We report a case of late-stage HCC who was successfully converted from unresectable to resectable with the help MDT treatment. Methods: A 60-year-old male patient admitted for portal vein tumor thrombus (PVTT) (stage C BCLC). The liver (with diameter 15cm) located in right lobe PVTT at main trunk and judged be insufficient volume left lobe. After discussion, comprehensive treatment performed downstage tumor, including Lenvatinib (12mg/d), checkpoint inhibitor (Sintilimab Injection, 200mg q3w) radiotherapy(GTV, 3.0Gyⅹ10 times) . Results: controlled well regressed vein. increased rapidly 415.7ml 661.5ml, which is sufficient hemi-hepatectomy. hemi-hepatectomy recovered well. pathological result showed that majority necrotic little middle-low differentiated central part PVTT. significant immunological change found tissues. continues take half-dose after discharger has survived 3 months surgery no recurrence. Conclusion: could HCC, convert resectable, as improve prognosis HCC.
